By submitting this form, I acknowledge and agree to the following terms: *I understand that one or both canvases will be returned to The Shoppes at Arbor Lakes by Monday, January 27th 2025. *I understand that the primary focus of my art should be heart-inspired. *I agree to keep the size of my artwork within the confines of the 12" x 12" canvas/es supplied. *I understand that the images I produce will belong to The Shoppes at Arbor Lakes and/or The Maple Grove Arts Center and may be sold to raise funds for The Maple Grove Arts Center as deemed appropriate. *I understand that I will make my artwork ready to hang with a wire across the back. *I agree to send a clear and web-ready ready image of my artwork to Lise Spence-Parsons, for website inclusion and marketing use. *I understand that I will not receive any form of payment for my artwork. By checking the box and submitting this form, I confirm my acceptance of the above terms.

General Liability *

INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise. I acknowledge that Maple Grove Arts Center and CBRE (The Shoppes at Arbor Lakes) and their directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf.
